We are seeking an experienced and driven Senior Community Liaison to join our Edenbrook Rochester community in Rochester, MN. As a key member of our team, you will play a crucial role in facilitating the smooth transition of patients from hospitals to our skilled nursing center.

Key Responsibilities:

  • Develop strong relationships with hospital social workers, physicians, and discharge planners to understand the needs of patients and communicate the services offered by our center.
  • Assess patient care needs and facilitate the transition of patients to our center, ensuring a seamless and efficient process.
  • Communicate effectively with the interdisciplinary team to ensure that patient needs are met and that we are providing the highest level of care.
  • Collaborate with local healthcare organizations to promote our services and build relationships that support the success of our community.

Requirements:

  • Knowledge of local hospitals and healthcare organizations, with ability to build relationships and communicate effectively.
  • Experience in sales and marketing, with a focus on generating referral sources and promoting services.
  • Excellent presentation and relationship-building skills, with the ability to communicate complex information in a clear and concise manner.
