Eden Senior Care, a leading healthcare management company, is seeking an experienced Social Worker to fill the role of Regional Hospital Liaison. This is an exciting opportunity for a motivated and results-driven professional to join our team.
We are looking for someone who is passionate about providing exceptional care and support to seniors and their families. As a Regional Hospital Liaison, you will be responsible for developing relationships with hospital social workers, physicians, and discharge planners to identify opportunities for program services.
The ideal candidate will have experience in case management, social work, or a related field, and possess excellent communication and interpersonal skills. You will be working closely with our interdisciplinary team to assess patient needs, develop personalized care plans, and facilitate smooth transitions into our centers.
We offer a competitive salary range of $60,000 - $80,000 per year, depending on experience, as well as a comprehensive benefits package, including health, dental, and vision insurance. Our generous PTO policy allows for flexible scheduling and work-life balance.
