Empower Individuals with Mental Health Challenges

HopeLink Behavioral Health is seeking a dedicated Intensive Transition Specialist to join our Community Readiness Support Program (CRSP) in Fairfax County. As a key member of our team, you will play a vital role in empowering clients to develop essential life skills and navigate their recovery journey.

Key Responsibilities:
  • Manage client caseloads and develop personalized recovery plans focused on individual goals and empowerment.
  • Design and deliver culturally relevant psycho-educational groups to enhance clients' health and wellness, independent living skills, and interpersonal skills.
  • Provide individual and group counseling services, including dual diagnosis support for co-occurring substance use disorders.
  • Develop and implement coping skills training, helping clients build healthy behaviors and manage their recovery.
  • Intervene in crisis situations, conduct safety assessments, and refer clients for emergency evaluations when necessary.
  • Coordinate care with case managers, healthcare providers, and other professionals to ensure holistic support.
  • Advocate for clients' needs and ensure access to essential services and resources.
  • Maintain meticulous clinical documentation in accordance with PRS policies, licensing regulations, Medicaid, and CARF standards.
  • Identify and screen for additional support needs, such as substance use treatment, medication management, and physical healthcare.
  • Collaborate with the team to continuously improve service delivery and client outcomes.
  • Participate in professional development opportunities to stay current in best practices.
Requirements:
  • Bachelor's degree in Social Work, Psychology, or a related human services field, with experience working with adults with mental health disorders.
  • Qualified Mental Health Trainee (QMHP-T) with a path to licensure; Qualified Mental Health Professional - Adults (QMHP-A) preferred.
  • Strong written and verbal communication skills, with a talent for clear and engaging presentations.
  • Self-motivated, highly organized, and adept at managing time effectively.
  • Ability to thrive in a collaborative team environment while also working independently.
  • Proven ability to build rapport with clients, staff, and external providers.
  • Experience with electronic health records (EHR) systems, preferably Credible EHR.
  • Strong computer skills and a willingness to learn new technologies.
  • Deep empathy and understanding of adult mental health challenges.
  • Must be fully vaccinated to meet contractual requirements.
