About the Role

We are seeking a highly skilled Registered Nurse to join our Admissions team at Heartland Hospice. As an RN Admissions Specialist, you will play a critical role in explaining our services to patients and families, assessing for appropriateness, and admitting patients to our hospice program.

Key Responsibilities
  • Explain our services to patients and families, resulting in the admission of appropriate patients
  • Utilize the nursing process to contribute to each patient's total plan of care
  • Visit patients and families in their place of residence or in the hospital to assess for appropriateness, explain services, monitor condition, or admit to hospice services
  • Participate in evening/weekend on-call as required
About You
  • Registered Nurse license in the state where the program is located
  • Bachelor's Degree and C.H.P.N. certification preferred
  • Minimum of 1 year general nursing experience, Hospice experience preferred
  • Minimum of 2 years Hospice Health Care or Medical Office experience, preferred
  • CPR Certification
