Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Care Division team at South Shore Health. As a Home Care Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and well-being.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
  • Develop and implement individualized care plans that address the unique needs and goals of each patient.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Conduct regular home visits to assess patient progress, provide education, and make adjustments to care plans as needed.
  • Document patient information, including progress notes, medication lists, and care plans, in a timely and accurate manner.
Requirements
  • Current licensure as a Registered Nurse in the state of Massachusetts.
  • Minimum of 1 year of experience in home health care or a related field.
  • Excellent assessment and communication skills, with the ability to work independently and as part of a team.
  • Ability to speak, read, and write English fluently.
Work Schedule

This is a part-time position, requiring a minimum of 1 weekend per month and 3 holidays per year. The schedule will be discussed in detail during the interview process.
