Diabetes Care Specialist

Job Summary:The Diabetes Care Specialist will provide comprehensive care to patients with diabetes, including education, support, and coordination of care. This role will work closely with a multidisciplinary team to ensure that patients receive the best possible care.
Responsibilities:- Conduct comprehensive assessments of patients with diabetes to identify their unique needs and develop individualized care plans.
- Provide education and support to patients and their families on diabetes management, including nutrition, medication, and lifestyle changes.
- Coordinate care with other healthcare providers to ensure that patients receive comprehensive and coordinated care.
- Monitor patients' progress and adjust care plans as needed.
- Participate in quality improvement initiatives to improve patient outcomes and care processes.
- Graduate degree in nursing or a related field.
- Current RN license in the state of Texas or a compact state.
- Three years of experience in diabetes care or a related field.
- Certification in diabetes education (CDE) or a related field.
- Excellent communication and interpersonal skills.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
- Bilingual (Spanish) or proficiency in a second language.
- Experience working with diverse patient populations.
- Knowledge of diabetes management and treatment options.
