Palliative Care Case Manager

The Case Manager, RN - Palliative is a critical role within the Mary Washington Healthcare Emergency Departments. This position is responsible for assessing, planning, implementing, and evaluating palliative patient referrals to ensure well-coordinated, multi-disciplinary programs of care.
Key Responsibilities- Identify and screen palliative care patients for care coordination with Emergency Department staff.
- Collaborate with providers to ensure seamless planning and care for palliative patients.
- Provide status updates regarding treatment plans with clinical partners as needed.
- Coordinate care for patients with clinical partners, helping patients and family members advance towards realistic and desirable outcomes.
- Manage patient/family/physician expectations, providing information and education on community resources.
- Assist with palliative-related education for patients, nursing staff, and physicians.
- Develop, coordinate, and communicate discharge plans with the patient, family members, and care team.
- Promote communication and coordination of care amongst care providers.
- Organize and expedite treatment plans, documenting assessment and overall discharge plan in the medical record.
- Valid RN Licensure in the Commonwealth of Virginia.
- Associate's or Bachelor's degree in Nursing, required.
- Certified Hospice and Palliative Nurse, required within 2 years of hire.
- Minimum of two years of experience in end-of-life care or hospice/palliative care, preferred.
