Palliative Care Social Worker Job Summary

The Palliative Care Social Worker is a vital member of our interdisciplinary care team, responsible for providing psycho-social assessments and medical social services to patients and their families. This role requires a strong understanding of palliative care principles and a commitment to delivering high-quality, patient-centered care.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' health status, risk factors, and psychosocial needs.
  • Develop and implement individualized care plans to address patients' physical, emotional, and spiritual needs.
  • Provide counseling and support to patients and their families to help them cope with serious illness and end-of-life care.
  • Collaborate with the interdisciplinary care team to ensure seamless coordination of care and effective communication with patients and families.
  • Participate in process improvement initiatives to enhance the quality and efficiency of palliative care services.
Requirements:
  • Bachelor's degree in Social Work from an accredited program, with LBSW licensure required OR Master's degree in Social Work from a school of social work accredited by the Council on Social Work Education.
  • AHA BLS Provider CPR certification required at hire.
  • Two years of experience in an acute care hospital setting preferred.
  • Knowledge and experience in palliative care, end-of-life care, and hospice care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse personalities and situations.
