Job Summary

We are seeking a skilled Critical Time Intervention Case Manager to join our team at Rappahannock Area Community Service Board. As a Critical Time Intervention Case Manager, you will provide intensive, time-limited case management services to individuals transitioning from institutional settings or homelessness to permanent housing.

Key Responsibilities
  • Provide field-based case management services to assist tenants in reaching treatment plan goals.
  • Offer crisis intervention services to enhance tenants' ability to independently problem-solve and manage their care.
  • Meet with tenants regularly to provide strengths-based case management and service coordination services.
  • Mediate and advocate on behalf of tenants to help them obtain and maintain stable housing, healthcare benefits, and other essential services.
  • Refer participants to community resources and follow up to ensure referrals are completed.
  • Assist clients in obtaining necessary documents, such as social security cards and birth certificates.
  • Obtain SOAR certification and assist clients in applying for disability benefits.
  • Track and document client progress.
Requirements
  • Bachelor's Degree in human services and at least one year of professional work experience in direct care to adults with serious mental illness.
  • Valid VA driver's license.
  • Qualified Mental Health Professional (QMHP) certification.
Working Conditions

This position involves working with clients who may have unsanitary behaviors, poor hygiene, and contagious illnesses. You will also be required to drive a vehicle and transport clients in inclement weather.
