Key Responsibilities:

  • Conduct resident assessments and care plan coordination.
  • Oversee the RAI process and ensure compliance with regulations.
  • Collaborate with the interdisciplinary team to develop and implement care plans.
  • Stay up-to-date with American Association of Nursing Assessment Coordinators (AANAC) requirements.

Requirements:

  • Licensed Practical Nurse (LPN) license.
  • AANC certification a plus.
  • Experience as an MDS Nurse.
