LPN Medication Care Manager Job Description

Sunrise Senior Living is seeking a highly skilled LPN Medication Care Manager to join our team. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care for our residents.

Responsibilities:
  • Medication Administration: Administer medications and treatments in a safe and organized manner, ensuring the highest degree of quality care for our residents.
  • Documentation: Accurately document medication administration, including any refusals or missed doses.
  • Communication: Maintain open communication with the Resident Care Director and Wellness Nurse regarding medication updates and resident concerns.
  • Medication Management: Restock medication carts, maintain the medication room, and ensure the availability of medications.
  • Resident Care: Provide resident care, including conducting Service and Health Updates, and participating in the development of Individualized Service Plans (ISPs).
  • Risk Management: Partner with the community team to ensure compliance with OSHA and other regulatory requirements, promoting a safe and healthy environment for our residents.
Qualifications:
  • LPN, LVN, or state/province specific licensed nurse credential.
  • CPR and First Aid certification.
  • Ability to make choices and decisions in the best interest of our residents.
  • Ability to react and remain calm in difficult situations.
  • Ability to handle multiple priorities and maintain a high level of organization.
