Job Summary

Housing and Services, Inc. is a nonprofit organization dedicated to ending chronic homelessness and providing supportive housing for vulnerable populations. We are seeking a skilled Care Coordinator to join our team at Kenmore Hall, a 325-unit supportive housing facility in the Flat Iron District.

The Care Coordinator will provide comprehensive case management services to residents, including coordination of care, supportive counseling, advocacy, and engagement/outreach. This role requires excellent communication skills, a strong understanding of community resources, and a commitment to a person-centered approach.

Key Responsibilities:

  • Coordinate care and services for residents, including medical compliance support and referral
  • Provide supportive counseling and advocacy to residents
  • Engage and outreach to residents to promote socialization and community involvement
  • Contribute to a program culture that is person-centered, holistic, and committed to continuous improvement

Requirements:

  • Minimum of a Bachelor's degree in psychology, social work, or related field
  • Experience working with people having histories of homelessness, serious mental illness, and/or substance use disorder
  • Excellent communication skills in written and spoken word
  • Computer literate and knowledge of community resources
