Job Summary

CareCollab is seeking a skilled Care Manager to lead the engagement and coordination of comprehensive care and services for high-utilization Medicaid members with chronic medical and behavioral health conditions.

This role is crucial in addressing the holistic needs of our members, including behavioral, medical, and social aspects. Key responsibilities include conducting assessments, developing individualized care plans, and ensuring timely documentation of services.

Key Responsibilities
  • Conduct initial and ongoing needs assessments to determine appropriate care management levels.
  • Ensure timely and accurate documentation using designated Care Management Portals and Electronic Health Records (EHRs).
  • Advocate for members with both internal and external service providers.
  • Promote wellness by linking members with tailored resources and services.
  • Educate caregivers on managing chronic conditions and preventive care.
  • Assist members in obtaining essential public benefits, including Medicaid and cash assistance eligibility, Social Security, SNAP, housing, legal services, employment and training supports.
  • Communicate effectively with members and families, considering language and cultural preferences.
  • Facilitate care planning meetings as part of a multidisciplinary team.
  • Identify and manage community resource referrals, ensuring effective engagement and follow-up.
  • Participate in discharge planning for hospitalized members, ensuring all post-discharge services are in place.
  • Attend ongoing staff development training to enhance care management skills.
  • Provide care coordination in members' homes and communities.
  • Collaborate with external stakeholders (i.e., hospitals, shelters, primary care providers) to achieve positive client outcomes.
Qualifications
  • Bachelor's degree in a related field OR a minimum of 2 years of relevant experience in medical or behavioral health services.
  • Excellent interpersonal skills to engage diverse audiences.
  • Strong organizational abilities to manage multiple priorities effectively.
  • Skills in conflict resolution and engagement.
  • Experience in Health Home or similar programs preferred.
  • Familiarity with multidisciplinary and field-based clinical work preferred.
  • Exceptional written and verbal communication skills.
  • Bilingual skills strongly preferred.
