About the Role

We are seeking a highly motivated and compassionate Hospice Liaison or Account Executive to join our team at Agape Care Group. As a key member of our organization, you will play a vital role in coordinating and generating new hospice business, identifying new markets, and maintaining existing service.

Key Responsibilities
  • Coordinate patient referrals and establish relationships with local hospitals and medical professionals to secure referrals.
  • Provide education to the community and referral sources on hospice services, highlighting the importance of compassionate care and support.
  • Work closely with agency personnel to ensure efficient and effective patient care, ensuring continuity of care and proper communication and documentation.
  • Regularly engage with patients, medical professionals, and referral sources to assure the highest level of care and service.
Requirements
  • A heart to serve patients and families, with a passion for providing the best possible care.
  • Minimum of an Associate Degree in a field of study or proven work experience in a health-related field.
  • 2 years of sales experience in a clinical care setting, with hospice experience preferred.
  • Reliable transportation and ability to sit, stand, bend, move intermittently, and lift at least 25lbs.
