Job Summary

We are seeking a skilled Nurse Practitioner to join our team at Ascension. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our staff to deliver exceptional patient outcomes.

Key Responsibilities
  • Provide Direct Patient Care: Deliver comprehensive nursing care to patients with behavioral health needs, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with Treatment Team: Participate in treatment planning and implementation, and contribute to the development of individualized care plans.
  • Conduct Bio-Psychosocial Assessments: Perform thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
  • Implement Nursing Plan of Care: Develop and implement individualized care plans, and evaluate patient progress and adjust plans as needed.
  • Lead Group Therapy Sessions: Conduct and/or co-lead group therapy sessions for patients, and provide education and support to patients and their families.
Requirements
  • Licensure and Certification: Current licensure as a Registered Nurse in the state of Illinois, and BLS certification.
  • Education: Diploma from an accredited school of nursing, or equivalent.
Preferred Qualifications
  • Experience: 1-2 years of experience working with geriatric patients with behavioral health diagnoses.
  • Willingness to Cross-Train: Willingness to cross-train and float throughout the hospital as needed.
