Job Title: Registered Nurse Care Coordinator/Case Manager

Ansible Government Solutions is seeking a highly skilled Registered Nurse Care Coordinator/Case Manager to support telehealth services for the Durham VA Healthcare System in Durham, NC.

Responsibilities:
  • Determine patient medical necessity and develop effective treatment plans.
  • Conduct care coordination and educate patients on their condition, treatment options, and benefits.
  • Ensure timely triage of incoming consults and update patient trackers with accurate information.
  • Contact patients with negative results and ensure consults are input and read for signature.
Requirements:
  • Ability to undergo a Government-sponsored background investigation (US Citizenship Required).
  • Associate Degree or Bachelor of Science in Nursing from an accredited program, college, or university, and hold a current license to practice as a Registered Nurse in any State.
  • Experience with RN activities in providing hematology/oncology care for a minimum of 2 years.
  • Telehealth Care Coordination/Case Manager experience required.
  • Specialization in Genetics.
  • Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) Certified.
  • Familiarity with medical terminology, hospital terminology, and/or clinics.

Technically proficient in the skills necessary to fulfill the government's requirements; will be provided training by the VA on appointment management, VA policy, and procedures.
