Key Responsibilities:
1. Coordinate all necessary systems and services to facilitate a well-organized, patient-centered care team.
2. Ensure delivery of quality, cost-effective healthcare for assigned patients.
3. Oversee treatment plans, collaborating with physicians, nursing staff, and other healthcare professionals.
4. Achieve quality patient outcomes within set timelines while optimizing resource utilization.
5. Conduct initial and ongoing patient assessments.
6. Manage and evaluate treatment outcomes.
7. Guarantee continuity of care through effective discharge planning.
8. Serve as a liaison for patients, families, healthcare team members, community resources, and staff as needed.
9. Uphold adherence to departmental policies and procedures.
Qualifications:
This position requires U.S. Citizenship for security clearance.
Must possess a degree in a health-related field, such as a Bachelor of Science in Nursing (BSN).
Active state Registered Nurse (RN) license is required.
Certification as a Case Manager (CCM) is highly preferred.
A minimum of three years of experience in case management is essential.
Physical Requirements:
All candidates must be capable of:
- Sitting, standing, walking, lifting, squatting, bending, twisting, and reaching overhead during the work shift.
- Lifting up to 50 lbs from floor to waist.
- Lifting up to 20 lbs.
- Carrying up to 40 lbs a reasonable distance.
- Pushing/pulling with a force of 30 lbs.
