About the Role

We are seeking a skilled Care Coordinator to join our team at Duke Health. As a key member of our healthcare team, you will play a vital role in ensuring the seamless transition of patients through the healthcare system.

Key Responsibilities
  • Monitor daily census and assignment to ensure all patients receive timely and effective care.
  • Perform comprehensive assessments to identify patient needs and develop personalized care plans.
  • Collaborate with healthcare teams to coordinate care transitions, including homecare, durable medical equipment, transportation, and skilled nursing or acute rehab facility placement.
  • Maintain accurate and timely documentation of patient information and care plans.
  • Participate in quality assurance and performance improvement activities to enhance patient outcomes and care coordination.
Requirements
  • BSN or MSW degree required.
  • 3 years of relevant experience in care coordination or a related field.
  • Case Management Certification (ACM, CCM, or ANCC) within 3 years of hire.
