Healthcare Social Worker

As a Medical Social Services Coordinator at Westminster Canterbury Richmond, you will play a vital role in assessing the psychosocial status and needs of our clients. You will work closely with the home health and hospice team to recognize and understand the social and mental stressors that impact client health and rehabilitation.
Key Responsibilities:
- Provide medical social services in accordance with a plan of care, including individual counseling, community resource planning, and crisis intervention.
- Assess the client's psychosocial status and develop a plan to address their needs.
- Collaborate with other team members to understand the social and emotional factors related to the client's health problems.
- Work with families as needed to provide support and resources.
- Prepare clinical and progress notes to document client care.
- Utilize and provide clients and families with appropriate community resources as needed.
- Participate in discharge planning to ensure a smooth transition for clients.
- Consult and coordinate services with primary care physicians and other disciplines as needed.
- Participate in in-service programs to stay up-to-date on best practices.
- Comply with agency policies and procedures related to infection control and safety.
