Job Overview

Healthcare Staffing Professionals, Inc. is seeking a dedicated Medical Case Worker to join our multidisciplinary team. The Medical Case Worker will play a vital role in our Interim Housing Outreach Program (IHOP), providing essential services to clients in licensed care facilities.

Key Responsibilities:
  • Conduct thorough in-person assessments of clients' medical, functional, and housing support requirements.
  • Provide continuous follow-up and services in alignment with the established care plan throughout the client's enrollment in the IHOP program.
  • Utilize gathered environmental data to recommend services that maximize benefits for each client.
  • Coordinate and oversee the execution of care plans, ensuring timely access to necessary medical services.
  • Regularly assess and update care plans based on clients' progress and evolving needs.
  • Maintain precise and current documentation of client assessments, care plans, interventions, and progress.
  • Collaborate with insurance providers, public agencies, and community organizations to advocate for clients' access to essential resources and services.
  • Adhere to legal and ethical standards regarding client confidentiality and information security.
  • Champion clients' rights and assist in overcoming barriers to accessing healthcare services.
  • Educate clients on their living conditions and self-care practices.
  • Support clients in understanding and following prescribed medication regimens and healthcare recommendations.
  • Provide information and referrals to relevant community resources and support services.
  • Work alongside healthcare professionals, social workers, and other stakeholders to ensure effective communication and coordination of care.
  • Facilitate communication among clients, facility operators, and the healthcare team, ensuring all parties are informed and involved in decision-making.
  • Participate in interdisciplinary meetings, contributing insights regarding clients' medical and psychosocial needs.
  • Prepare reports summarizing clients' medical and social situations, highlighting significant changes or concerns.
  • Make referrals and collaborate with various public and private health, mental health, or other agencies to address clients' personal and social challenges.
  • Develop tailored care plans for low-risk clients in partnership with the patient, healthcare team, and other relevant parties.
Qualifications:
  • Minimum one year of experience providing services to patients and clients in healthcare settings.
  • Bachelor's degree from an accredited institution and one year of professional casework experience in interviewing, counseling, and assisting clients with social issues.
  • A valid California Class C Driver's License or the ability to utilize alternative transportation methods for job-related functions.
  • Comprehensive knowledge of medical terminology, healthcare systems, and available community resources.
  • Exceptional communication and interpersonal skills, with the ability to build rapport with diverse individuals and foster effective relationships.
  • Strong organizational and problem-solving skills, coupled with empathy and a genuine desire to assist others.
  • Familiarity with relevant laws, regulations, and ethical standards in healthcare.
  • Proficiency in utilizing computer systems and healthcare software for documentation and data management.
Certifications:
  • A valid California Class C Driver's License or the ability to utilize alternative transportation methods for job-related functions.
  • Current American Heart Association Basic Life Support (BLS) certification.
  • Successful completion of the Live Scan and Health Clearance process.
Additional Skills:
  • Experience in homeless healthcare.
  • Knowledge of complex care management services for high-need Medi-Cal beneficiaries.
  • Demonstrated teaching and communication skills with both clinical and non-clinical staff.
  • Expertise in trauma-informed care and harm reduction principles.
  • Familiarity with community mental health and substance use programs and resources.
  • Ability to perform light physical tasks, including lifting up to 10 lbs., bending, and considerable walking.
