The Clinical Social Worker II plays a pivotal role in implementing and executing emerging initiatives in social services at L.A. Care Health Plan. This position ensures that all special programs and initiatives function in accordance with the organization's mission, values, and strategic goals.
Key Responsibilities- Provides a wide variety of professional social work services within the Social Services Department to support special programs and initiatives.
- Performs project-based activities, including program support to ensure successful development and implementation of existing and emerging initiatives.
- Participates in professional committees, meetings, and conferences related to special projects and initiatives in Social Services.
- Manages projects assigned to ensure compliance with program policy and acts as a Subject Matter Expert.
- Develops and maintains effective working relationships with other services areas within the organization and external partners.
- Interviews individuals, their families, and/or caregivers to complete psychosocial assessments and identifies and analyzes the member's needs for services.
- Develops plans to attain identified goals with the help of the member and provides consultation and information to clients, stakeholders, internal, and external departmental professionals.
- Master's Degree in Social Work
- At least 3 years of experience working with adults, seniors, persons with disabilities, individuals with chronic health conditions, and/or dually diagnosed individuals in a healthcare setting or social agency.
- Knowledge of managed care and an understanding of Medi-Cal and Medicare programs.
- Excellent administrative and organizational skills, strong customer service skills, and proficiency with Microsoft Office.
- Managed care experience, preferably in a care/case management role.
- Previous experience with a health plan.
- Bilingual in one of L.A. Care Health Plan's threshold languages is highly desirable.
This position requires a light physical workload and the ability to work in a fast-paced environment.
