- Develop and implement comprehensive care plans, working closely with interdisciplinary teams to meet the unique needs of each resident.
- Coordinate the generation and transmission of Minimum Data Set (MDS) reports, ensuring timely and accurate data submission.
- Provide leadership and participate in quality assurance initiatives, monitoring and improving care processes to ensure the best possible outcomes for our residents.
- Collaborate with colleagues to develop and deliver in-service training programs, enhancing the skills and knowledge of our care team.
To be successful in this role, you will possess a Nursing Degree from an accredited college or university, with a minimum of two years of experience as a supervisor in a hospital, nursing care community, or other related healthcare facility. You will also have a minimum of six months of training experience in rehabilitative and restorative nursing practices.
A current State RN license is required, and you will be knowledgeable of general, rehabilitative, and restorative nursing and medical practices, procedures, regulations, and guidelines governing long-term care.
