Position Summary:

The Case Manager plays a crucial role in supporting residents of transitional housing by facilitating access to essential services and advocating for their needs. This position is responsible for assisting residents in overcoming obstacles to secure permanent housing and developing personalized housing strategies.

The ideal candidate thrives in a collaborative environment with staff and volunteers, demonstrating the ability to respond swiftly to emergencies and challenges. Critical thinking skills are essential for guiding residents towards actionable solutions that foster stability. The candidate must be capable of working autonomously while maintaining clear and comprehensive communication with team members. A strong organizational skill set, attention to detail, and the ability to engage effectively with community resources are vital for success in this role. The successful candidate will exhibit enthusiasm, resourcefulness, and reliability, along with exceptional interpersonal skills and a commitment to serving vulnerable populations. This position aims to equip program residents with the necessary tools and support to achieve lasting housing stability.

Key Responsibilities:

  • Manage a caseload of up to 20 residents, conducting weekly interviews with each individual.
  • Collaborate with residents to create a comprehensive housing plan that includes goals related to income, health, and housing search, while monitoring progress and providing necessary interventions.
  • Maintain precise documentation in case files and relevant systems, including interactions with residents, family members, and social service agencies.
  • Prepare and submit monthly reports and other required documentation.
  • Coordinate with fellow program staff regarding resident interactions within the program.
  • Assist residents in navigating community systems, including attending appointments and providing hands-on support to overcome barriers.
  • Advocate and coordinate with referral agencies to ensure residents receive appropriate services.
  • Facilitate problem-solving sessions with residents, including conducting group discussions or one-on-one coaching.
  • Perform additional duties as assigned by the Director of Ozanam Manor.
  • Engage in and exemplify the organization's Culture of Philanthropy, ensuring that volunteers feel welcomed and valued.

Preferred Qualifications:

  • Strong communication skills.
  • Professional demeanor: Business casual attire.
  • Proficiency in Spanish is preferred.
  • Familiarity with psychiatric, medical, and human service delivery systems.
  • Commitment to volunteer engagement and appreciation, as well as participation in the Culture of Philanthropy.

Education and Experience Requirements:

  • Bachelor's degree in a related field is preferred.
  • Proficient in standard office software (Microsoft Office).

Physical Demands:

  • Ability to stand, walk, and lift up to 25 pounds.
  • This role requires frequent, direct interaction with staff, volunteers, and clients.
