Job Title: Assessment Coordinator

St. Vincent's Hospital Westchester is seeking a highly skilled Assessment Coordinator to join our team.

Job Summary

The Assessment Coordinator will be responsible for conducting clinical assessments of Adult Homes Class members and overseeing the assessment sector of our Supported Housing program.

Key Responsibilities
  • Conduct thorough clinical assessments of Adult Homes Class members
  • Oversee the assessment sector of our Supported Housing program
  • Collaborate with program staff to ensure seamless service delivery
  • Develop and implement assessment protocols and procedures
  • Monitor and evaluate program effectiveness
Requirements
  • Master's Degree in social services or behavioral health related field
  • Professional license (LCSW or LMHC)
  • Minimum three years post-graduate experience in Mental Health setting
  • Minimum two years supervisory experience
  • Excellent written and verbal communication skills
  • Excellent interpersonal and organizational skills
  • Proficient in Microsoft Office Suite and database navigation
