Job Summary

The Transplant Social Worker will play a vital role in evaluating the psychosocial needs of patients and their family support systems, coordinating appropriate treatment plans for identified patient populations.

Key Responsibilities
  • Evaluate the psychosocial needs of patients and family support systems
  • Coordinate appropriate treatment plans for identified patient populations
  • Act as a liaison between the facility and external resources
  • Facilitate timely treatment plans and follow-up to anticipated post-acute interventions
  • Provide crisis intervention and support
  • Assist the Case Manager in facilitating patient movement across the continuum of care
  • Identify and track barriers to patient throughput
Requirements
  • Current Social Work License to practice in the State of Texas
  • Master of Social Work degree
  • 2 or more years of Social Work transplant experience
  • Demonstrate analytical and critical thinking abilities with pro-active decision-making and negotiation skills
