Job Summary

We are seeking a highly skilled Clinical Transplant Social Work Specialist to join our team. The successful candidate will be responsible for assessing the needs of adult and pediatric patients undergoing transplants and providing support to help them access medical care and adhere to treatment regimens.

Key Responsibilities

  1. Assess patient needs and develop personalized care plans to ensure seamless transitions from pre-transplant to post-transplant care.

  2. Provide counseling and education to patients and their families on transplant-related issues, including financial and environmental difficulties.

  3. Collaborate with healthcare teams to ensure timely and effective communication of patient needs and progress.

  4. Develop and maintain relationships with community resources and healthcare providers to facilitate access to care and support services.

Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work (MSW) or a related field.

  • 2+ years of experience in transplant social work or a related field.

  • Strong assessment and counseling skills, with the ability to work with diverse patient populations.

  • Excellent communication and interpersonal skills, with the ability to work effectively with healthcare teams and community resources.

Benefits

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, as well as a comprehensive benefits package, including medical, dental, and vision coverage, 401(k) retirement plan, and paid time off.
