The Clinical Care Coordinator provides support in managing clients' medication routines, ensuring safety, adherence, and understanding of prescribed medications. This role emphasizes education, safety monitoring, and medication organization, without direct medication administration.

Key Responsibilities:
  • Develop and maintain a network of sub-contracted service providers, including home care agencies, transportation providers, and hospice services.
  • Schedule and conduct pre-admission assessments of prospective members, and provide input on decisions regarding applicants and members.
  • Communicate regularly with members, including quarterly phone calls and in-person visits as needed.
  • Manage quality monitoring initiatives and participate in the Quality Monitoring Committee.
  • Network with community organizations to enhance awareness of the continuing care at home program.
  • Utilize AOD to document interactions with members, family members, or other member associates.
Requirements:
  • Graduate of an approved RN program.
  • Familiarity with Windows operating systems, MS Office, and email software.
  • Ability to work remotely and/or share office space when necessary.
  • Strong commitment to quality care and service, with a sensitivity to the needs and concerns of older adults.
