About the Role:
Venza Care at Regency Rehabilitation and Healthcare Center is seeking a dedicated and detail-oriented RN with MDS certification to join our team as an MDS Coordinator. As a key member of our interdisciplinary team, you will play a pivotal role in ensuring the quality of care and compliance with federal and state regulations.
Key Responsibilities:
- Conduct comprehensive assessments of new and existing residents using the MDS tool.
- Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team.
- Ensure timely transmission of MDS assessments to relevant authorities.
- Train and guide nursing staff on MDS procedures and updates.
- Monitor and audit clinical records for accuracy and completeness.
- Collaborate with healthcare providers to optimize patient care and outcomes.
Qualifications:
- Active RN license with MDS Certification
- Proven experience in clinical assessment and MDS coordination.
- In-depth knowledge of federal and state regulations related to MDS and care planning.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills to interact with staff at all levels.
