Medical Care Coordinator

As a Medical Care Coordinator at Neighborhood Health, you will play a vital role in ensuring that patients receive high-quality, safe, and patient-centered medical care. This position requires strong communication and organizational skills, as well as the ability to work effectively with a multidisciplinary team.
Key Responsibilities
• Respond to patient phone calls, questions, and requests for care in coordination with the medical patient care team.
• Inform patients of lab and diagnostic results and the need for follow-up care.
• Administer medications and immunizations as needed.
• Provide education to patients on various health topics.
• Assist with ordering and maintaining clinical supplies, onsite medications, and equipment.
• Participate in clinic quality improvement and risk management activities.
Requirements
• Current RN or LPN license in the state of Virginia.
• A minimum of one year of experience in a healthcare setting.
• Strong organizational and communication skills.
• Knowledge of Microsoft Office and Microsoft Excel.
• Fluency in Spanish a plus.
