Key Responsibilities- Provide whole-person care to patients in their homes, focusing on their physical, emotional, and social needs.
- Work collaboratively with the healthcare team to develop and implement individualized care plans.
- Conduct thorough assessments of patients' conditions and develop effective care strategies.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' progress and adjust care plans as needed.
- Graduate of an accredited Licensed Practical/Vocational Nurse program.
- Washington Licensed Practical Nurse upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- Ability to obtain an NPI number within 30 days of starting in the position.
- 1 year of recent hospital, nursing home, or home health experience.
