Job Summary

Cynet Health is seeking a skilled Home Health RN to administer high-quality nursing care to patients in their place of residence. As a Home Health RN, you will work independently to assess patient needs, develop care plans, and provide services or treatments requiring substantial and specialized nursing skill.

Key Responsibilities
  • Conduct thorough nursing assessments on admission and re-evaluate patient needs on succeeding visits
  • Initiate, complete, and update patient care plans using Medicare OASIS Forms
  • Provide services or treatments requiring substantial and specialized nursing skill according to the written patient care plan
  • Manage all therapies, including LPN, PT, OT, ST, and CNA
  • Counsel patients and family members/significant others in meeting nursing and related needs
  • Process orders and notify physicians of patient needs and changes in condition
  • Conduct patient care conferences on assigned patients
Requirements
  • Graduate of an accredited school of nursing
  • License in good standing
  • One year of recent experience in Home Health
  • AHA BLS certification
