Job Summary

We are seeking a skilled Case Manager to join our team at Volunteers of America-Greater New York. The successful candidate will work with clients to assess their strengths and needs, plan a course of action, and link them to services. This is a key role in our Case Management Model, and the right individual will be responsible for ensuring clients receive appropriate, competent, and professional information and advice.

Key Responsibilities
  • Manage the client intake and assessment process to orient the client to the program and produce a comprehensive assessment of client service needs.
  • Develop with the client a mutually agreed-upon individual service plan reflecting assessment findings and goals.
  • Meet regularly with the client to assist them in obtaining needed services/benefits to achieve their individual service plan goals.
  • Document case management activities in accordance with agency and funder guidelines and procedures.
Requirements
  • Associates Degree or 60 college credits courses in Human Services field, or High School Diploma or GED with 1 year of relevant shelter experience.
  • Demonstrated ability in verbal and written communication, including basic computer skills.
