Job Title: Homebase Program Case Manager

RiseBoro Homecare Inc. is seeking a skilled and compassionate Homebase Program Case Manager to join our team. As a key member of our Homebase Program, you will play a vital role in providing housing assistance and holistic case management services to individuals and families at risk of homelessness.

Key Responsibilities:
  • Coordinate services and activities to provide program participants with necessary tools to sustain their housing and prevent homelessness
  • Work with clients, household members, supervisors, and supplementary staff to execute the service plan
  • Establish goals and actions that address the immediate housing crisis and underlying causes of the crisis
  • Monitor and evaluate program participants' progress on the implementation of their service plan
  • Confer with supervisors and program directors to determine the necessity of financial subsidies
  • Prepare documentation for in-house subsidy applications on behalf of clients
  • Provide referrals for clients to access community supports, public benefits, and entitlements
  • Maintain accurate record keeping via case files and designated computer databases
  • Engage in the arrangement, coordination, monitoring, and delivery of services to Homebase program participants
  • Ensure that program participants' rights are protected
Requirements:
  • Bilingual English/Spanish strongly preferred
  • Experience working in non-profit organizations and commitment to social justice
  • Excellent communication skills and ability to advocate
  • Ability to work independently and collaborate with a team
  • Entrepreneurial spirit, ability to problem-solve, and build relationships
  • Associate's degree required; Bachelor's degree preferred
