Homebase Case Manager

The Homebase Case Manager position at RiseBoro Homecare Inc. is responsible for providing housing assistance and holistic case management services to program participants. The ideal candidate will have a strong commitment to social justice and excellent communication skills.
Key Responsibilities
Coordinate services and activities to provide program participants with necessary tools to sustain their housing and prevent homelessness
Work with clients, household members, supervisors, and supplementary staff to execute the service plan
Establish goals and actions that address the immediate housing crisis and the underlying causes of the crisis
Monitor and evaluate program participants' progress on the implementation of their service plan
Confer with supervisors and program directors to determine the necessity of financial subsidies
Prepare necessary documentation for in-house subsidy applications on behalf of clients
Provide referrals for clients to access community supports, public benefits, and entitlements
Maintain accurate record keeping via case files and the designated computer database
Engage in the arrangement, coordination, monitoring, and delivery of services to Homebase program participants
Ensure that program participants' rights are protected
Requirements
Bilingual English/Spanish required
Experience working in non-profit organizations and a commitment to social justice
Excellent communication skills and ability to advocate
Ability to work independently and collaborate with a team
Entrepreneurial spirit, ability to problem solve, and build relationships
Associate's degree required; Bachelor's degree preferred
