Job Summary

VIA Health Partners, a top 10 nationally ranked provider of end of life care, is seeking an experienced Nurse Practitioner Preceptor to join our team. As a community-based, not-for-profit hospice & palliative care provider, we pride ourselves on serving ALL patients and families needs regardless of their ability to pay or their medical complexity.

Key Responsibilities
  • Mentor, implement, and evaluate clinical training and competencies of advanced practice providers.
  • Provide policy and procedure development, dissemination, education, and compliance to established policies, protocols, and documentation/billing requirements.
  • Disseminate and communicate clinical practice changes.
  • Analyze APP agreed upon data to identify trends and make recommendations for modifications of practice based on analysis.
  • Assist in the orientating, planning, evaluating, and setting of objectives for onboarding and ongoing education for Advanced Practice Provider.
  • Oversee and develop orientation/onboarding for Advanced Practice Provider for palliative and hospice with Chief Medical Officer.
  • Ensure ongoing updates to orientation as standards and best practice guidelines change.
  • Conduct regular/on-going meetings with designated Advanced Practice Provider to address clinical competency gaps, and concerns.
  • Complete routine visits with Advanced Practice Provider to ensure compliance to clinical standards, protocols, and quality, exceptional care.
  • Partner with Branch leadership with structured feedback regarding the progress, success, and challenges of Advanced Practice Provider.
  • Audit Advanced Practice Provider's documentation and competency in the EMR.
  • Advise and ensure APP s' involvement and communication with the IDGs to meet patient and or family representative goals of care.
Requirements
  • 10 years of relevant experience in hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, or medical surgical.
  • A master's degree in nursing.
  • Certification in hospice and palliative care nursing required within six months of hire if not already certified.
  • Experience in training in palliative care and symptom management and/or recent hospital experience.
  • Knowledge of program management is desired.
  • Current, active, and unrestricted license NC and SC license as Registered Nurse (RN), and official recognition as Advanced Practice Provider from NC and SC Board of Nursing.
  • Current and unrestricted DEA and Basic Life Support required.
