El Hogar Community Services Inc. is seeking highly skilled Mental Health Service Coordinators to join their dynamic team in Sacramento County.

The estimated annual salary for this role is between $19 and $23 per hour, depending on experience and qualifications.

About the Job

This exciting opportunity will see you working with adults 18 and older, providing mental health and related resources across various programs, including CORE-Mariposa, CORE-Marigold, Community Justice Support Program, and Sierra Elder Wellness Program. Each program caters to unique populations and requires a dedicated approach.

Job Responsibilities:

  • Deliver behavioral health services, substance abuse support, and case management services aligned with the Psychosocial Rehabilitation Model.
  • Conduct assessments, service plan development, housing support, skills training, group facilitation, crisis intervention, and individual/group therapy.
  • Manage caseloads, maintaining regular contact with clients as defined by program contracts.
  • Prepare assessments, progress notes, and clinical documents adhering to agency standards and HIPAA regulations.
  • Ensure consumers' assigned caseload meets county authorization requirements.

Essential Qualifications:

  • A High School diploma or equivalent required, supplemented by two years of full-time direct mental health experience.
  • Associate's degree with six years of direct mental health experience, or Bachelor's degree with four years of direct mental health experience.
  • Proficiency in Contact Management systems, typing at least 35-45 words per minute, and ability to assimilate county-required billing software (Smartcare).
