Job Overview:

We are seeking a skilled Mental Health Service Coordinator to join our team at Northern California Behavioral Health System. In this role, you will work closely with patients, families, and healthcare professionals to ensure comprehensive care and support. As a key member of our multidisciplinary team, you will contribute to creating a safe and therapeutic environment that promotes healing and recovery. Responsibilities:
  1. Contribute to the development and implementation of individualized treatment plans, working collaboratively with patients, families, and healthcare providers to address complex mental health needs.
  2. Provide direct patient care, including assessment, intervention, and education, to promote optimal outcomes and quality of life.
  3. Coordinate with interdisciplinary teams to ensure seamless transitions and continuity of care, collaborating with social workers, therapists, and other healthcare professionals as needed.
  4. Maintain accurate and detailed records of patient interactions, treatment plans, and progress, utilizing electronic health records and other documentation tools.
  5. Develop and facilitate educational programs and workshops to empower patients, families, and community members with knowledge and skills to manage mental health conditions effectively.
Requirements:

Education/Licensure:
  • High school diploma or equivalent required.
  • CPR certification (obtained during initial orientation).
  • Handle with Care training obtained during initial orientation.
Skills and Abilities:
  • Familiarity with medical and psychological terminology, human behavior principles, and cultural competence.
  • Excellent communication and interpersonal skills, with ability to establish trust and rapport with diverse populations.
  • Ability to work independently and as part of a collaborative team, prioritizing tasks and managing time effectively.
  • Good judgment, problem-solving, and decision-making skills, with attention to detail and accuracy.
  • Physical stamina to perform job requirements, including standing, walking, and lifting up to 50 pounds occasionally.
