Link Jobs seeks a Primary Care Specialist to provide high-quality care to homebound patients in Georgia. This is a rewarding opportunity for a skilled Nurse Practitioner to utilize their expertise in assessment, evaluation, and disease prevention.

  • Responsibilities:
  • Assess and evaluate patient condition through comprehensive physical examinations and medical history review.
  • Reconcile medications and screen for depression and substance abuse.
  • Develop personalized care plans to address chronic and transitional care needs.

This role requires a current NP license in Georgia, at least 1 year of clinical experience, and proficiency with Electronic Health Records (EHR). The ideal candidate will possess excellent communication skills and be able to work independently.

  • Requirements:
  • Current NP license in Georgia
  • At least 1 year of clinical experience
  • Clinical background in adult, family, or geriatric specialties
  • Experience working with EHR systems
  • Effective communication and interpersonal skills
  • Ease of use with smartphones and email
  • Reliable transportation
