Job Description

The Phoenix At Milton is hiring a Geriatric Care Specialist to provide high-quality care and services to our residents in assisted living and memory care neighborhoods.

About the Role

As a Geriatric Care Specialist, you will be responsible for providing physical and emotional support to a consistent group of residents, while maintaining a safe and comfortable home-like environment.

Key Responsibilities

  • Develop and update Personalized Service Plans (PSPs) in collaboration with families and healthcare professionals
  • Implement hydration schedules and ensure resident meal attendance sheets are completed
  • Comply with all resident rights and initiate activities as needed
  • Conduct daily checks on assigned residents for cleanliness, clothing, and continence issues
  • Use proper sanitary procedures and universal precautions
  • Check assigned resident rooms daily for general cleanliness and neatness
  • Assist with laundry and common area cleaning, including bathrooms
  • Complete ADLs for assigned residents according to PSPs
  • Respond promptly to security system alerts and resident call bells

Educational Requirements

  • High school education preferred
  • Previous experience working with elders or disabled individuals
  • Must be at least 18 years old to perform personal care tasks
  • Knowledge of Assisted Living industry and Dementia care or Skilled Nursing
  • Ability to work flexible hours, including weekends and evenings

Compensation and Benefits

We offer a competitive salary of $40,000 per year, plus benefits that include health insurance, retirement plan, and paid time off.
