About This RoleThis opportunity is for a highly skilled Senior Care Physician to join our team in delivering high-quality, patient-centered care to seniors. As a key member of our multidisciplinary care team, you will work closely with other healthcare professionals to provide coordinated care and improve patient outcomes.
Responsibilities- Evaluate and treat center patients in accordance with standards of care.
- Follow level of medical care and quality for patients and monitors care using available data and chart reviews.
- Assist in the coordination of patient services, including but not limited to specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
- Act as an active participant and key source of medical expertise with the care team through daily huddles.
- Help Regional Medical Director and Center Administrator in setting a tone of cooperation in practice by displaying a professional and approachable demeanor.
- Complete all medical record documentation in a timely manner working with a quality-based coder to optimize coding specificity.
- Follow policy and protocol defined by Clinical Leadership.
- Meet with RMD about quality of care, review of outcome data, policy, procedure and records issues.
- Participate in potential growth opportunities for new or existing services within the Center.
- Participate in the local primary care "on-call" program of CenterWell as needed.
- Assure personal compliance with licensing, certification, and accrediting bodies.
CenterWell Senior Primary Care provides proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment and more. Our unique care model focuses on personalized experiences, taking time to listen, learn and address the factors that impact patient well-being.
We Offer- A competitive salary of $199,400 - $298,300 per year.
