Job Title: Social Worker

We are seeking a highly skilled and compassionate Social Worker to join our team at CN Guidance and Counseling Services. As a Social Worker, you will play a vital role in providing innovative and person-centered integrated clinical treatment, counseling, housing, and support services to individuals living with mental health and substance use conditions.

Key Responsibilities:
  • Conduct diagnostic assessments to determine treatment priorities and develop individualized treatment plans.
  • Implement crisis services to prevent deterioration and promote adaptive functioning.
  • Share information about our agency and its services to empower clients and other stakeholders in treatment decisions.
  • Help clients become effective consumers of mental health and substance abuse services.
  • Maintain client stability through ongoing treatment relationships and assess changes in client functioning when necessary.
  • Regularly assess treatment progress with supervisors and be open to recommendations for improvement.
  • Assist clients in developing realistic expectations from treatment and understanding the need for cooperation with fee and insurance responsibilities.
  • Strengthen clients' ability to understand and utilize therapy and find resources after treatment has ended through proper discharge planning and termination processes.
  • Establish and maintain accurate clinical case records, including treatment plans, progress notes, and treatment plan reviews.
  • Exchange information with other agencies to benefit clients and ensure compliance with regulations and coordination of services.
  • Perform outreach to clients and home visits as necessary.
  • Participate in required internal and external meetings to represent client needs and agency goals.
  • Utilize and understand the value of supervision to increase practice skills, promote better service for clients, and preserve accountability to the agency.
  • Supervise graduate and undergraduate students when assigned, incorporating client needs, agency goals, education, and university requirements.
  • Facilitate data sharing with individuals to facilitate partnered decision-making and keep clients informed of progress.
Requirements:
  • Master's degree in Social Work required.
  • Active registration as a Licensed Certified Social Worker in New York State or a plan to become certified.
  • Prior successful field work experience under proper supervision.
