Your Responsibilities:

As a Mental Health Case Manager, you will provide direct, hands-on support to a diverse range of individuals, fostering genuine, therapeutic, and trusting relationships while being sensitive to their experiences and trauma.

In this role, you will be responsible for completing necessary paperwork and documentation, as well as providing referrals and case coordination for individuals receiving services, which may include mental health, medical, financial, social, educational, vocational, housing, and legal support.

Your duties will be performed in accordance with Rule 79, which encompasses documentation, functional assessments, crisis plans, releases, and individual community support plans, all conducted within scheduled timeframes and performance standards.

You will interact with clients frequently, at least once a month or as needs arise.

Additionally, you may develop relationships with county and state staff, family members, employers, and other agencies to support the individuals receiving our services.


Over time, this position will allow you to grow and enhance your personal skill set while contributing to the development of this expanding division within our organization.

Qualifications:
To be considered for this role, you should possess a BA or BS in human services or a related field; a BSW must hold a valid and current social work license.

Additionally, you must have a valid driver's license, adequate insurance coverage, and access to a vehicle.
