We are seeking an experienced and dedicated Advanced Care Coordinator, RN to join our team. This is a fantastic opportunity to work with a dynamic organization and make a meaningful difference in the lives of our members.
About the RoleThis position involves providing community-based care coordination/management for our members in Delaware. As an Advanced Care Coordinator, you will be responsible for assessing, planning, coordinating, implementing, and evaluating care for eligible members with complex healthcare needs.
You will travel to members' homes, nursing facilities, and other community-based settings to complete face-to-face needs assessments and provide ongoing support via telephonic contact. Your goal will be to ensure that members receive the most effective and efficient care possible, while also considering their social determinants and psychosocial needs.
Key Responsibilities- Travel to members' homes and other settings to complete needs assessments and provide support
- Assess, plan, coordinate, implement, and evaluate care for eligible members with complex healthcare needs
- Collaborate with healthcare providers, caregivers, and other stakeholders to develop comprehensive care plans
- Provide education and resources to members and caregivers on health care needs, benefits, and services
- Develop and maintain relationships with community partners and resources to ensure seamless care coordination
To be successful in this role, you will need:
- A current Registered Nurse license in the state of Delaware
- At least 2 years of experience in case management and discharge planning, including experience working with geriatric populations
- Experience completing assessments, developing service plans, and collaborating with healthcare providers
- Strong communication and interpersonal skills, with the ability to build strong relationships with members, caregivers, and healthcare teams
- Ability to work independently and as part of a team, with a focus on delivering high-quality patient-centered care

SalaryThe estimated annual salary for this position is $85,000 - $105,000, depending on experience and qualifications.
