Job Description

We are seeking a skilled Patient Care Coordinator - RN to join our team at CAI. As a member of our community-based care coordination team, you will play a critical role in ensuring the best possible outcomes for our patients.

About the Role

The Patient Care Coordinator - RN will be responsible for assessing, planning, and implementing individualized care plans for members in Delaware. This involves conducting face-to-face needs assessments, coordinating care across multiple services, and working closely with healthcare teams to address physical and behavioral health needs.

Key Responsibilities
  • Travel to various locations to conduct face-to-face needs assessments and develop comprehensive care plans
  • Collaborate with healthcare teams to ensure seamless care coordination and optimal patient outcomes
  • Coordinate care across multiple services, including chronic care management and community healthcare
  • Educate patients and caregivers on available resources and services to support their health and well-being
  • Document all case management activities and interventions in electronic health records
Requirements
  • Certified Registered Nurse (RN) licensure in the state of Delaware
  • At least 2 years of experience in case management and discharge planning, preferably in a facility setting
  • Strong communication and interpersonal skills, with ability to work effectively with diverse populations
  • Proficiency in PC-based word processing and database documentation software
  • Able to travel regularly within assigned territory and work flexible hours to meet patient needs
Preferred Qualifications
  • Certified Case Manager (CCM)
  • Licensed Bachelor's Social Worker (LBSW), Licensed Master's Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW)
  • Experience working with HIV/AIDS population, behavioral health population, developmental disabilities population, Medicare, and Medicaid

Estimated Salary: $85,000 - $110,000 per year, depending on experience and qualifications.
