Job Description

We are looking for a skilled and dedicated Acute Care Coordinator RN to join our team. As a key member of our interdisciplinary team, you will play a vital role in ensuring seamless transitions for patients moving through the care continuum.

The ideal candidate will have a strong background in medical-surgical nursing, with a focus on acute care and discharge planning. You will work closely with patients, families, and healthcare providers to develop and implement effective care plans that meet individual needs and promote positive outcomes.

In this role, you will also participate in quality improvement initiatives, analyze data to identify trends and areas for improvement, and contribute to the development of policies and procedures related to care management.

Key Responsibilities
  • Conduct thorough assessments of patient needs and develop personalized care plans.
  • Collaborate with multidisciplinary teams to ensure timely and effective transitions throughout the care continuum.
  • Analyze data to identify trends and areas for improvement in care management processes.
  • Participate in quality improvement initiatives to enhance patient care and satisfaction.
Requirements
  • Bachelor's degree in Nursing required.
  • Active RN license in DE or Compact State required.
  • At least 2 years of medical-surgical nursing experience in an acute care hospital setting.
  • Minimum of 1 year of Care Management experience preferred.
  • Case Management Certification (CMC) within 18 months of eligibility preferred.
