Advanced Palliative Care Specialist

**Job Overview**
We are seeking an experienced Advanced Palliative Care Specialist to join our team at Resource 1 Homecare Staffing. As a Palliative Nurse Practitioner, you will play a vital role in providing compassionate care to patients with serious illnesses and their families.
Salary: $100,000 - $112,000 per year
Location: New Castle County
Job Description:
As a Palliative Nurse Practitioner, you will be responsible for assessing patients' physical, psychological, emotional, spiritual, and social needs. You will prescribe medications and treatments to manage symptoms, document patient visits and discussions using our EMR system, communicate findings and recommendations to the care team, participate in discharge planning, and stay current on goals of care conversations, Advance Health Care Directives, and DMOST.
Responsibilities:
- Assess patients' physical, psychological, emotional, spiritual, and social needs
- Prescribe medications and treatments to manage symptoms
- Document patient visits and discussions using our EMR system (Netsmart)
- Communicate findings and recommendations to the care team
- Participate in discharge planning to ensure safe transitions across the care continuum
- Stay current on goals of care conversations, Advance Health Care Directives, and DMOST
Qualifications:
- Master's degree and certification as a Certified Registered Nurse Practitioner (CRNP)
- Current Delaware nursing license
- CPR certification
- Minimum of two years of nursing experience, preferably in hospice or palliative care
- Certification (or willingness to pursue) as an Advanced Nurse Practitioner in Hospice and Palliative Care
