Community Services Assistant Team Supervisor

As a Community Services Assistant Team Supervisor, you will play a vital role in ensuring the well-being of program participants by fostering an environment that promotes independence and positive relationships within a secure learning atmosphere.
Compensation:
$19.50/hour
Work Schedule:
Monday-Friday 7:30am-3:30pm
Creative Works is dedicated to supporting adults with disabilities, both in their homes and within the community. With a legacy dating back to 1967, we have established exceptional community services that provide essential daytime activities for individuals with disabilities and autism.
Our facilities are vibrant spaces equipped with resources such as art studios, fully-equipped kitchens for collaborative meal preparation, lush gardens, technology centers, and fitness areas.
KEY RESPONSIBILITIES:
- Advocate for assigned individuals by attending meetings, ensuring medical, dental, and therapy appointments are scheduled and attended, and fostering family and community relationships.
- Attend to personal care needs of individuals, which may include lifting and assisting with mobility, as well as maintaining hygiene standards.
- Provide transportation for individuals as necessary, utilizing personal vehicles for appointments and activities.
- Facilitate transitions between assignments as requested, including moving individuals between residences and day programs.
- Conduct intake and orientation for new participants.
- Assist the Team Leader with scheduling and operational functions.
- Identify and connect participants with internal and external resources.
- Complete all required reports, including Quality Assurance summaries, in a timely manner.
- Maintain relationships with families, community resources, and other service providers to ensure effective service coordination.
- Verify documentation to support billing processes within specified time frames.
PHYSICAL REQUIREMENTS:
- Ability to perform standing and sedentary tasks for extended periods.
- Capability to perform physical tasks, including reaching, bending, stretching, and lifting.
- Ability to lift up to 50 pounds, with training provided on proper lifting techniques.
QUALIFICATIONS:
- High School Diploma or GED required.
- Previous experience in a related field is preferred.
- Valid US driver's license with a satisfactory driving record as per agency standards.
- Proficiency in reading, writing, and speaking English sufficient to perform essential job functions.
- Basic math skills necessary for job performance.
- Willingness to obtain required training within the agency's specified timeline, including DSP, CPR/FA, and Welle.
- Strong organizational and prioritization skills are essential.
