POSITION SUMMARY:
As a Community Services Assistant Team Leader, you will play a vital role in ensuring the well-being of program participants by creating an environment that promotes independence and positive relationships.

Compensation:
$19.50/hour

Work Schedule:
Monday-Friday 7:30am-3:30pm

Creative Works is dedicated to supporting adults with disabilities in various settings, including home, work, and community environments.

Since its inception in 1967, Creative Works has been at the forefront of providing exceptional community services that facilitate essential daytime activities for adults with disabilities and autism.

Our facilities are equipped with a wealth of resources, including art studios, fully-equipped kitchens for collaborative meal preparation, flourishing gardens, technology centers, and fitness areas.

We are currently seeking a full-time Assistant Team Leader for our Community Services Day Program. If you are a direct support professional eager to enhance your leadership capabilities, we encourage you to consider this opportunity.


KEY RESPONSIBILITIES:


1. Advocate for assigned individuals by attending all relevant meetings, ensuring that medical, dental, and therapy appointments are scheduled and maintained, and fostering family and community relationships.

2. Attend to personal care needs of individuals, which may involve lifting and assisting individuals in and out of wheelchairs, utilizing a Hoyer lift, and maintaining hygiene standards.

3. Provide transportation for individuals as needed, using a personal vehicle for appointments and work-related activities.

4. Facilitate the transfer of individuals between assignments as requested, ensuring smooth transitions.

5. Conduct intake and orientation for new participants, ensuring they feel welcomed and informed.

6. Support the Team Leader with scheduling and organizational tasks.

7. Identify and connect participants with internal and external resources to meet their needs.

8. Submit all necessary reports, including Quality Assurance summaries and outcomes, to the Quality Assurance Coordinator.

9. Assist in service coordination by maintaining relationships with families, community resources, and other involved providers.

10. Verify documentation to support billing within established time frames.


PHYSICAL REQUIREMENTS:
Ability to perform standing and sedentary tasks for extended periods.
Ability to execute range of motion tasks, including reaching, bending, stretching, stooping, and lifting.
Ability to lift up to 50 pounds (all personnel receive training in proper lifting techniques).

QUALIFICATIONS:
High School Diploma or GED.
Previous experience in a related field is preferred.
Valid US driver's license with a satisfactory driving record as per agency standards.

Must possess the ability to read, write, and speak English sufficiently to perform essential job functions. Basic math skills are also required.
Must be willing to obtain necessary training within the Agency's specified timeline, including but not limited to: DSP, CPR/FA, and Welle.
