Job Summary

This is an exciting opportunity to make a difference in the lives of older adults by bringing home-based medical care to complex, chronic patients. As a member of our Landmark team, you will help establish and maintain effective relationships with patients, caregivers, and healthcare professionals to provide high-quality care.

The ideal candidate will have Certification through the American Academy of Nurse Practitioners (AANP) or American Nurses Credentialing Center (ANCC), current, unrestricted NP licensure in State of Practice, and Nurse Practitioner must actively hold, unrestricted RN licensure in State of Practice.

Key Responsibilities
  • Establishes and maintains effective relationships with patients, caregivers, and healthcare professionals.
  • Performs initial comprehensive assessments on newly enrolled patients and provides ongoing care.
  • Develops patient management care plans upon enrollment and updates them as needed.
  • Consults with Physician Leaders and coordinates with PCPs in these decisions.
  • Comfortable with basic procedures of episodic nursing care, including IV placement, blood draws, injections, foley catheters, nasal packing, basic wound care, etc.

Benefits and Requirements

We offer a competitive salary range of $105,000 - $135,000 per year, depending on experience. You'll also receive benefits, bonuses, and opportunities for growth and development within our organization.

To qualify for this role, you'll need 2+ years of clinical experience, preferably in primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar settings. A strong knowledge of CPT, ICD-10, and HCPC codes, as well as clinical standards of care, is essential.
