Job Summary

As a Case Manager, you'll integrate healthcare services for clients from pre-admission to post-discharge. You'll coordinate with healthcare teams, evaluate patient responses, and monitor outcomes. Your expertise will ensure that clients receive comprehensive care, tailored to their unique needs.

Responsibilities
  • Coordinate and sequence client care to ensure seamless transitions
  • Evaluate patient responses and monitor outcomes
  • Collaborate with healthcare teams to provide comprehensive care
  • Develop and implement care plans that meet client needs
  • Monitor and report on client progress
Requirements
  • Bachelor's degree in nursing or related field
  • Minimum of one to two years of professional nursing experience
  • Current RN license with the State Board of Nursing
  • Ability to work independently and as part of a team
  • Strong communication and problem-solving skills
